Hello, my name is Rosanna Peralta. I'm a social worker.

Can care happen at home or in a care community?

Hospice care focuses on comfort, and it often happens where a person lives. That can be a private home, an assisted living community or a nursing home.

Visits may come from nurses, aides, social workers and trained volunteers, with a doctor guiding the plan of care. Some hospices also run inpatient units, and hospice care can take place in a hospital. A short stay away from home, called respite care, can give family caregivers a rest.

Call the office if you have questions about your visit or your care. This is general information, not personal instructions. Your hospice team can tell you what applies to you.

Thanks for watching.
