Hello, my name is Sylvie. I'm a care coordinator.
Why do so many people see me today?
At a complex care clinic, several helpers see you in one visit. Seeing them all together can save your family extra trips.
You might meet a doctor, a nurse, a dietitian and a therapist. Each one checks something different, like how you eat, grow, breathe or move. Some might check equipment, like a wheelchair or braces. You can tell your grown-up when you need a stretch. Your grown-up can ask for a short break.
Later, the team often talks together and shares one plan with your grown-up.
Thanks for watching!
